Problem

Istio’s AuthorizationPolicy resource is the mesh’s primary L7 access control mechanism. Each policy defines who can reach a given workload based on the source principal (the mTLS SPIFFE identity of the calling pod), request headers, HTTP method, path, and JWT claims embedded in the Authorization header. Istio’s Envoy sidecars enforce these policies in the data plane, evaluating every inbound request against the set of policies that match the destination workload. The design gives platform engineers fine-grained, cryptographically backed access control across thousands of service-to-service calls without modifying application code.

That expressiveness comes with a persistent attack surface. Any feature that allows policy conditions to reference HTTP headers must accurately parse multi-value and repeated headers. HTTP allows multiple instances of the same header name in a request, and HTTP/2 permits comma-separated values within a single header field. Whenever the policy engine and the application disagree on how to interpret these ambiguous cases, an attacker can craft a request that the policy allows while the application processes it as if the policy had never existed.

CVE-2026-26308 (disclosed April 2026, CVSS 7.5) is the third major iteration of this class in Istio’s history. In certain configurations, Istio’s RBAC enforcement checked only the first value of a multi-value HTTP header when evaluating request.headers conditions. An AuthorizationPolicy blocking requests where X-Custom-Role equals admin could be defeated by sending X-Custom-Role: regular-user,admin — the policy read regular-user, allowed the request, and the backend application received the full header and acted on admin. Affected versions are Istio 1.29.0, 1.28.0 through 1.28.4, and 1.27.0 through 1.27.7. Patches shipped in 1.29.1, 1.28.5, and 1.27.8 alongside the public advisory.

CVE-2026-22771 (Envoy Gateway, April–May 2026, High) describes a Lua filter sandbox escape in Envoy Gateway. Lua scripts configured through EnvoyFilter patches or HTTPRoute extensionRefs could access internal proxy state that should be isolated from the Lua execution context, including TLS private key material and downstream and upstream authentication tokens. An operator-supplied Lua script — or a Lua script injected through a misconfigured extension pulled from an untrusted source — could exfiltrate TLS session keys from the Envoy process. Envoy Gateway 1.5.7 and 1.6.2 close the escape by tightening the Lua sandbox’s access to the Envoy C++ API surface.

Header manipulation vulnerabilities recur in Istio’s authorizaton layer. CVE-2021-39156 was a path normalization bypass that allowed requests to skip policies matching specific path prefixes. CVE-2022-39278 exploited header manipulation to bypass RBAC conditions. CVE-2026-26308 follows the same structural pattern: a new capability in AuthorizationPolicy (more expressive header matching), an edge case in how Envoy’s HeaderMap handles repeated header entries, and a policy bypass. Each time Istio adds expressiveness to its authorization model, multi-value header handling must be re-validated across every code path that reads from the header map.

The upstream fix for CVE-2026-26308 was not coordinated perfectly with the Istio advisory release. The Envoy maintainers merged a patch to envoy/source/common/http/header_map_impl.cc that changed how HeaderMapImpl::getExisting aggregates multi-value header entries; the commit was publicly visible on the Envoy repository before Istio’s advisory and patch releases were tagged. Security researchers monitoring Envoy’s HeaderMap commit history could identify the fix, infer the Istio vulnerability, and exploit it against unpatched clusters before the advisory reached the istio-security-announce mailing list. CVE-2026-22771 followed the same pattern: the Lua sandbox restriction commit appeared in the public envoy-gateway repository before the patch releases were tagged. Target systems: Istio 1.27.x < 1.27.8, 1.28.x < 1.28.5, 1.29.x < 1.29.1; Envoy Gateway < 1.5.7 (1.5.x branch) and < 1.6.2 (1.6.x branch).

Threat Model

  1. Multivalue header bypass by external client. An external client sends X-Custom-Auth: allowed-value,denied-operation as a comma-separated single header, or repeats the header as two separate lines. Istio’s AuthorizationPolicy evaluates only allowed-value and permits the request. The backend application, which iterates all values, receives and acts on denied-operation, performing an action the policy was intended to block.

  2. Lua credential exfiltration via untrusted EnvoyFilter. A platform engineer deploys a third-party EnvoyFilter manifest found in a public Helm chart or blog post. The manifest installs a Lua HTTP filter. On an unpatched Envoy Gateway, the Lua script uses the CVE-2026-22771 escape to call into internal Envoy C++ bindings, reads TLS private key material from the proxy’s in-memory certificate store, and exfiltrates it via an outbound HTTP request to an attacker-controlled endpoint. The exfiltrated key can be used to decrypt captured TLS sessions or impersonate the service.

  3. Patch-gap attacker exploiting public Envoy commit history. An attacker reads the header_map_impl.cc multivalue fix on GitHub, understands that Istio versions prior to 1.29.1 bundle the unfixed Envoy, and begins scanning the internet for Istio control-plane healthz endpoints (/healthz/ready on port 15021 of Istio ingress gateways) to enumerate managed clusters. The istio-version response header or the pilot-discovery version endpoint reveals the running version. The attacker then crafts multivalue header requests against auth-protected internal services exposed through the ingress gateway, exploiting the bypass before the advisory is public.

  4. JWT claim bypass via repeated Authorization headers. Similar to the request.headers multi-value bug, Istio’s JWT claim evaluation in AuthorizationPolicy may behave inconsistently when a request carries two Authorization: Bearer <token> headers — one with a valid, low-privilege token and one with an expired or invalid token. Depending on which header the JWT validation filter processes first and which value reaches the requestAuthentication principal extraction, a request that should be denied on claim grounds may be allowed.

If any of these scenarios succeed, the blast radius depends on what the bypassed policy was protecting. For internal microservice RBAC, a header bypass grants caller-level access to a restricted API without valid mTLS identity enforcement — the attacker can read or mutate data scoped to an internal service account. For credential exfiltration via the Lua escape, the blast radius extends to every TLS session the compromised Envoy instance has terminated: an attacker with the private key can retroactively decrypt any captured traffic if perfect forward secrecy was not enforced.

Configuration / Implementation

Upgrading Istio

Patch to a fixed version before any other hardening steps. The multivalue header fix in CVE-2026-26308 is in the Envoy binary bundled with Istio; all other policy changes are advisory without it.

For production clusters, use a canary upgrade strategy: upgrade the control plane first, then roll out the new sidecar version namespace by namespace using a rolling restart:

Only roll to the next namespace after confirming traffic is healthy and proxy-status shows all proxies synchronized. The old and new sidecar versions interoperate during the rollout window.

Header Normalization in AuthorizationPolicy

After patching, audit every AuthorizationPolicy that uses request.headers conditions. Prefer exact match over prefix or suffix matching; exact match is less susceptible to normalization edge cases.

Test that multi-value headers are rejected after upgrade. HTTP/2 clients join repeated headers with commas; HTTP/1.1 clients send repeated header lines. Test both forms:

Prefer DENY-Default AuthorizationPolicy

action: ALLOW with specific conditions is fail-closed: a request that matches no ALLOW rule is denied. action: DENY with specific conditions is fail-open: a request that matches no DENY rule is allowed. For sensitive workloads, structure policies as ALLOW-only with a blanket namespace-level deny:

Use from.source.principals (mTLS SPIFFE identities) as the primary authorization signal. mTLS identities are cryptographically bound to workload certificates issued by Istio’s CA and cannot be spoofed by injecting a header. Header-based conditions should be supplementary checks on top of an mTLS-verified principal, not the primary authorization gate.

Envoy Gateway Lua Filter Hardening

Audit all Lua filters deployed through EnvoyFilter or HTTPRoute extensionRefs. The following command surfaces every EnvoyFilter that patches an HTTP filter layer:

Flag any Lua script that calls streamInfo():downstreamSslConnection() or accesses ssl() objects on stream handles — these are the patterns that CVE-2026-22771 uses to reach certificate data. Upgrade Envoy Gateway to 1.5.7 or 1.6.2:

Where Lua filters exist solely to perform header inspection or simple request routing decisions, prefer CEL-based authorization in Istio AuthorizationPolicy or Envoy Gateway’s BackendTLSPolicy and native header matching. CEL expressions run in Envoy’s native extension framework with no script sandbox boundary to escape.

Validating AuthorizationPolicy Enforcement

Use istioctl’s experimental authorization check to simulate requests against a specific pod and port before deploying policy changes to production:
